Heart and circulatory disease, also known as cardiovascular disease (CVD), causes a quarter of all deaths in the UK and is the largest cause of premature mortality in deprived areas. CVD is the single biggest area where the NHS can save lives over the next 10 years. Early detection and treatment of CVD can help patients live longer, healthier lives.

Through local analysis we anticipate there are ~20k people living in Camden who are considered “undiagnosed and/or at high-risk of CVD and CVD related conditions” (data from HealthEIntent Tool, CVDPrevent and ratified by primary care).

Early identification of people at risk and proactive management has the potential to deliver a significant financial return on investment to the Camden Health and Care economy, considering the wider costs of CVD to the national economy are estimated at £15.8 billion.

This programme aims to provide early detection and engagement for people who are undiagnosed and/or high-risk of developing CVD.

Why should I get my practice involved?

This is a fully funded initiative with financial incentive for your practice as detailed in your contract with Camden Health Partners (CHP). Working with iPLATO, SMS messages will be sent to eligible patients registered at your practice, who have a mobile number and have opted in for SMS communication.   

It's simple and easy to support:

Step 1: Register your practice.

Step 2: Create a slot type and allocate a sufficient number of admin clinic slots (5 minute duration) on your practice clinical system.

Step 3: Provide list of eligible patients NHS numbers (run already made EMIS report) and the name of admin clinic slots to our secure NHS mail inbox.

Step 4: Generate the patients blood forms and send them a Swiftqueue link to book a blood test appointment at a hospital.

Step 5: Review results and calculate QRisk.

Step 6: Provide University College London Partners with anonymised patient data via secure NHS Mail for them to complete a project evaluation. 

Step 7: Provide a list of NHS numbers to iPLATO of the patients who have completed their health check (blood test) as this will be utilised to confirm the practices financial claim with CHP.

What is a health check?

An NHS Health Check takes about 20 to 30 minutes and are performed by a health care professional – often a nurse or healthcare assistant. The aim of this program is to help reduce practice workload by turning this into a digital process.

We have developed a questionnaire that will ask patients about their lifestyle and family history, ask for their height and weight, and support them while they take their own blood pressure. Once completed they will then be directed to get a blood test to complete the health check.

By increasing the capacity of how many people are able to take a Health Check we are hoping more health conditions can be picked up, as NHS Health Checks identifies conditions that can lead to preventable deaths in the UK.

How will this process work?

  • iPLATO Healthcare will be sending messages by app or SMS to eligible patients registered at eligible practices across the Camden area.
  • Once patients complete the questionnaire, the GP practice will be informed via the admin clinic on their clinical system. The practice will generate the patient's blood forms and send them a Swiftqueue link to book a blood test appointment at a hospital.
  • Patient blood test results and questionnaire responses are readcoded back to the clinical system for the practice to review and calculate each patients QRisk score.
  • The practice will provide a report to iPLATO of the patients who have completed their health check.

Data Processing and Data Sharing Summary

  • The practice provides a list of eligible patients NHS numbers to our secure NHS mail inbox. 
  • iPLATO will facilitate the ongoing extraction of client demographic data via the practice clinical system phonebook, for which the data will be minimised as far as possible.
  • All data is processed within the HSCN network.
  • Only patients who have consented to receive SMS will be contacted.
  • iPLATO Healthcare acts as a data processor for this service.
