If patient is coded for GDM:
Your GP has identified that you have a history of gestational diabetes mellitus. This means you are eligible for the FREE NHS Diabetes Prevention Programme. For more info & to register, please ring 033 3577 3010. You will need the following details: Your NHS no. _____ HbA1c: ___ & test date:__/__/____.
Follow up SMS (same text sent to everyone):
Dear [NAME], [GP PRACTICE NAME] sent you a letter about your reserved place on the free NHS Diabetes Prevention Programme. Call 033 3577 3010 to accept your place on the programme now. If you have already signed up, or have decided not to take part, please ignore this text.
1. The purpose of the data sharing is to enable the sending of text messages and letters to each eligible patient. The letter will contain details about the NDPP programme they have been referred to, followed by a reminder SMS prompting them to self-refer.
2. Data will be retrieved via a secure connection to each practice’s clinical system.

Yes - Once you have submitted consent, iPlato will contact you to obtain the eligible patient list and other required information. Once the search has been conducted, the information will be sent to the iPlato nhs.net email address.
Yes, although we would encourage a discussion with the practice to understand the reasons for this and explore any support we could provide the practice to ensure continued participation.
